Authorization to Repair

I hereby authorize Elite Collision to repair my vehicle according to the estimate that was presented to them or prepared by them at the time the vehicle was left at the shop. If no estimate has been presented or prepared, I give Elite Collision permission to disassemble the damaged parts so that they may prepare a thorough estimate. I understand that the estimate is subject to change due to hidden damage or parts price differences. If there is an insurance company paying for this claim, I give Elite Collision permission to negotiate recommended repair procedures with that insurance company on my behalf. In the event that insurance company does not agree to recommended repairs, I will be notified by Elite Collision. I give Elite Collision permission to drive my vehicle on public roads for the purpose of test driving or transport to sublet vendors. If at any time I decide not to have my vehicle repaired I understand that I am responsible for any labor charges or non-returnable parts that have been incurred at that point. I understand that payment in full is due upon completion of the repairs. I understand that Elite Collision is not responsible for personal items or valuables left in my vehicle.

Directions to Pay

I herby authorize the insurance company or liable party that is paying this claim, to pay Elite Collision directly on my behalf. I understand that if the payment is mistakenly made to me instead, that I am responsible for making that payment to Elite Collision. I understand that Elite Collision is under no obligation to release the vehicle until payment has been received in full.
